Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It collects and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses to examine the effect of treatment across trials of different levels of pragmatism.
Background
Pragmatic trials provide real-world evidence that can be used to make clinical decisions. The term "pragmatic", however, is not used in a consistent manner and its definition and evaluation need further clarification. Pragmatic trials should be designed to guide clinical practice and policy decisions, rather than to prove a physiological or clinical hypothesis. A pragmatic study should strive to be as close as it is to the real-world clinical practice that include recruitment of participants, setting, design, implementation and delivery of interventions, determination and analysis results, as well as primary analyses. This is a major distinction between explanation-based trials, as described by Schwartz and Lellouch1 which are designed to test the hypothesis in a more thorough way.
Studies that are truly practical should not attempt to blind participants or the clinicians as this could lead to bias in estimates of treatment effects. Furthermore, pragmatic trials should focus on outcomes that are important to patients, such as quality of life or functional recovery. This is particularly important for trials that involve invasive procedures or have potentially serious adverse effects. The CRASH trial29, for instance was focused on functional outcomes to evaluate a two-page case report with an electronic system to monitor the health of patients in hospitals suffering from chronic heart failure. Similarly, the catheter trial28 focused on urinary tract infections that are symptomatic of catheters as its primary outcome.
In addition to these aspects the pragmatic trial should also reduce the trial procedures and data collection requirements in order to reduce costs. Furthermore, pragmatic trials should seek to make their results as applicable to real-world clinical practice as is possible by making sure that their primary method of analysis follows the intention-to treat approach (as described in CONSORT extensions for pragmatic trials).
Many RCTs that do not meet the requirements for pragmatism however, they have characteristics that are in opposition to pragmatism, have been published in journals of varying kinds and incorrectly labeled pragmatic. This can result in misleading claims of pragmaticity and the usage of the term should be standardized. The development of a PRECIS-2 tool that can provide an objective, standardized assessment of pragmatic features is a first step.
In a practical trial the goal is to inform policy or clinical decisions by demonstrating how an intervention would be implemented into routine care. This differs from explanation trials, which test hypotheses about the causal-effect relationship in idealized conditions. Therefore, pragmatic trials might have lower internal validity than explanatory trials and may be more susceptible to bias in their design, conduct, and analysis. Despite their limitations, pragmatic studies can be a valuable source of data for making decisions within the healthcare context.
The PRECIS-2 tool assesses the level of pragmatism that is present in an RCT by assessing it across 9 domains ranging from 1 (very explanatory) to 5 (very pragmatic). In this study, the recruitment, organisation, flexibility: delivery and follow-up domains were awarded high scores, but the primary outcome and the procedure for missing data were not at the limit of practicality. This suggests that it is possible to design a trial that has good pragmatic features without harming the quality of the results.
However, it's difficult to determine how practical a particular trial is since pragmatism is not a binary quality; certain aspects of a study can be more pragmatic than others. Moreover, protocol or logistic modifications during the course of an experiment can alter its score in pragmatism. Koppenaal and colleagues found that 36% of 89 pragmatic studies were placebo-controlled or conducted prior to licensing. The majority of them were single-center. This means that they are not very close to usual practice and can only be called pragmatic if their sponsors are tolerant of the lack of blinding in these trials.
Furthermore, a common feature of pragmatic trials is that researchers try to make their results more relevant by analyzing subgroups of the sample.
