LPN Brain Sheet Template for a 4-Patient Shift (Long-Term Care and Subacute)

Free download: grab the Free Clinical Day Checklist from Shift Ready Studio, a one-page printable for the night before and the morning of clinical or shift.

A brain sheet (also called a report sheet) is the one page a nurse keeps in a pocket all shift: who the residents are, what is due when, and what needs to be reported. New LPNs who use one finish med pass on time more often and give cleaner end-of-shift report.

What a good LPN brain sheet includes

  • Room, name initials, code status, allergies, and diet for each resident
  • A timeline of scheduled meds, fingersticks, and treatments by hour
  • Fall risk and skin check boxes for each round
  • PRN given, time, and reassessment time
  • New orders, calls to the provider, and family updates
  • A short SBAR line per resident for end-of-shift report

How to use it on shift

  1. Fill the top section during report before you start med pass.
  2. Cross off each timed item as you finish it, so nothing slides.
  3. Write every PRN and reassessment time the moment you give it.
  4. Use the SBAR line to give report in under two minutes per resident.
