Many serious accidents have happened because a machine was turned on when it was supposed to be completely turned off to allow for maintenance or repair. These accidents are usually the result of stored energy that isn't properly released. Lock-out tag-out (LOTO) systems can help avoid these accidents by making sure that machines remain completely off until the authorized individual releases them.
The LOTO system is comprised of a set of locks, tags and other devices that are used to secure, isolate or shut off the device from electrical power. All "affected", "authorized" and "qualified" personnel should be taught about the use of these devices. These devices include, but aren't restricted to chains, locks and padlocks. Each device is uniquely identified and labeled with the date and time at which it was applied along with the name and address of the authorized person and the reason for the lockout/tagout (repairs, maintenance or repairs, etc.). It is imperative to state clearly that these devices are not to be removed until the LOTO procedure is complete.
