Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that facilitates research on pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2 permitting multiple and varied meta-epidemiological studies to compare treatment effects estimates across trials with different levels of pragmatism and other design features.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. The term "pragmatic", however, is not used in a consistent manner and its definition and measurement require further clarification. Pragmatic trials are designed to inform clinical practices and policy decisions, not to prove a physiological or clinical hypothesis. A pragmatic trial should aim to be as close as is possible to the real-world clinical practice, including recruitment of participants, setting up, implementation and delivery of interventions, determining and analysis results, as well as primary analyses. This is a major distinction between explanatory trials as defined by Schwartz & Lellouch1, which are designed to prove a hypothesis in a more thorough way.
Trials that are truly pragmatic must not attempt to blind participants or the clinicians as this could result in bias in the estimation of treatment effects. The trials that are pragmatic should also try to attract patients from a variety of health care settings so that their results can be compared to the real world.
Furthermore, trials that are pragmatic must be focused on outcomes that matter to patients, such as the quality of life and functional recovery. This is particularly relevant when trials involve surgical procedures that are invasive or may have serious adverse effects. The CRASH trial29, for example focused on the functional outcome to compare a 2-page case-report with an electronic system for the monitoring of patients admitted to hospitals with chronic heart failure, and the catheter trial28 used urinary tract infections caused by catheters as the primary outcome.
In addition to these characteristics, pragmatic trials should minimize the trial procedures and requirements for data collection to reduce costs. In the end, pragmatic trials should aim to make their findings as applicable to current clinical practices as they can. This can be accomplished by ensuring that their analysis is based on the intention-to treat method (as described within CONSORT extensions).
Many RCTs which do not meet the requirements for pragmatism but contain features contrary to pragmatism have been published in journals of various types and incorrectly labeled pragmatic. This can lead to misleading claims of pragmatism, and the term's use should be made more uniform. The creation of the PRECIS-2 tool, which provides a standard objective assessment of pragmatic characteristics is a good initial step.
Methods
In a pragmatic study the aim is to inform clinical or policy decisions by showing how an intervention can be integrated into routine care in real-world settings. This is different from explanatory trials that test hypotheses about the cause-effect connection in idealized situations. Therefore, pragmatic trials could be less reliable than explanatory trials and might be more susceptible to bias in their design, conduct and analysis. Despite their limitations, pragmatic research can provide valuable information to make decisions in the context of healthcare.
The PRECIS-2 tool scores an RCT on 9 domains, ranging from 1 to 5 (very pragmatic). In this study the areas of recruitment, organization and flexibility in delivery, flexible adherence and follow-up were awarded high scores. Additionally, studies that are pragmatic can present challenges in the gathering and interpretation of safety data. This is due to the fact that adverse events are usually self-reported, and therefore are prone to delays, errors or coding variations. It is important to improve the accuracy and quality of outcomes in these trials.
Results
While the definition of pragmatism does not require that all trials are 100% pragmatic, there are some advantages of including pragmatic elements in clinical trials. These include:
Enhancing sensitivity to issues in the real world, reducing study size and cost and allowing the study results to be faster translated into actual clinical practice (by including patients who are routinely treated). However, pragmatic trials can also have drawbacks. For instance, the appropriate type of heterogeneity can help a trial to generalise its findings to a variety of settings and patients. Their framework included nine domains, each scoring on a scale ranging from 1-5, with 1 being more informative and 5 indicating more practical. The use of these terms in titles and abstracts could indicate a greater understanding of the importance of pragmatism however, it is not clear if this is manifested in the content of the articles.
Conclusions
As the value of real-world evidence becomes increasingly widespread, pragmatic trials have gained popularity in research. They are randomized studies that compare real-world care alternatives to experimental treatments in development. They are conducted with populations of patients closer to those treated in regular medical care. This approach could help overcome the limitations of observational studies, such as the limitations of relying on volunteers and the lack of accessibility and coding flexibility in national registries.
Pragmatic trials also have advantages, like the ability to draw on existing data sources, and a greater probability of detecting meaningful differences than traditional trials. However, they may still have limitations that undermine their reliability and generalizability. 프라그마틱 순위 in certain trials could be lower than expected due to the healthy-volunteering effect, financial incentives, or competition from other research studies. Practical trials are often restricted by the necessity to enroll participants on time.
