LPN Cheat Sheet for Long-Term Care: One Page to Run Your Whole Shift

Free download: grab the Free Clinical Day Checklist from Shift Ready Studio, a one-page printable for the night before and the morning of clinical or shift.

New LPNs in long-term care and subacute rehab do not need more facts. They need one place to keep the facts they already have. A shift cheat sheet is that place: one page per shift with your residents, times, and to-dos, so you stop flipping through scraps of paper.

What belongs on an LPN shift cheat sheet

  • Each resident's room, code status, allergies, and diet
  • Scheduled med pass times, fingersticks (AC and HS), and treatments
  • PRNs given, with the time you owe a reassessment
  • Vital signs due and any abnormal values to follow up on
  • Falls precautions, turning schedule, and skin checks due
  • New orders, labs, and provider calls to make or return
  • Anything you must hand off at report

How to use it every shift

  1. Fill in the resident list during report, before your first med pass
  2. Block your times: med pass, fingersticks, treatments, rounds, charting
  3. Check off tasks as you go, and circle anything that did not get done
  4. Write change-of-condition details in SBAR order so the call is ready
  5. Use the circled items as your end-of-shift report
