LPN SBAR Template for Change of Condition and Shift Report (Long-Term Care and Subacute)

SBAR stands for Situation, Background, Assessment, and Recommendation. It is the standard way nurses organize a call to the provider or a hand-off to the next shift, so the listener gets the important facts in under a minute. New LPNs who use a written SBAR template call with fewer pauses, get clearer orders, and forget fewer details at report.

What a good LPN SBAR template includes

  • Situation: who you are, the resident, the room, and the one-sentence reason for the call
  • Background: diagnosis, code status, allergies, recent changes, and current relevant meds
  • Assessment: a full set of vital signs, pain score, mental status compared with baseline, and what you see
  • Recommendation: what you are asking for (an order, an evaluation, a transfer, or a time to recheck)
  • Read-back lines for new orders, plus who you notified (supervisor, family) and the time

How to use it on shift

  1. Gather vitals, the MAR, and the latest notes before you pick up the phone.
  2. Fill each SBAR box in short phrases, not full sentences.
  3. Read the sheet aloud on the call so nothing gets skipped.
  4. Write the orders you receive, read them back, and note the time and provider name.
  5. Use the same layout for end-of-shift report so the next nurse hears it in the same order.
