LPN Care Plan Template for Nursing Students (NANDA, SMART Goals, Interventions, Evaluation)

Care plans eat clinical weekends. Most of the time is not the thinking, it is rebuilding the same layout every week: diagnosis, related-to, as-evidenced-by, a goal with a real date, interventions, rationales, and an evaluation. A reusable care plan template keeps the structure fixed so your effort goes into the resident, not the formatting.

The care plan structure most LPN programs expect

  1. Nursing diagnosis (NANDA-I format): the problem, related to (R/T) the cause, as evidenced by (AEB) what you actually assessed. Example: Risk for falls R/T impaired mobility and diuretic use, or Impaired skin integrity R/T immobility AEB stage 2 pressure injury on the sacrum.
  2. SMART goal: specific, measurable, achievable, relevant, and timed. "Resident will remain free of falls through the end of the shift on 11/4 at 2300" beats "Resident will be safe."
  3. Nursing interventions: independent actions written as "The nurse will..." with a frequency. Example: The nurse will round on the resident every hour and keep the call light within reach.
  4. Rationales: one sentence per intervention, with a citation to your textbook or drug guide.
  5. Evaluation: measured against the goal. Goal met, partially met, or not met, with the data that proves it.

Quick tips that save points

  • Pull the AEB straight from your assessment. If you did not see it or chart it, it does not belong there.
  • Match the diagnosis to your screening scores. A high fall risk score usually calls for a fall diagnosis, and a low Braden score usually calls for a skin integrity diagnosis.
  • Keep interventions inside the LPN scope your program teaches.
  • Never put resident names, room numbers, or other identifying details on anything that leaves the facility.
